- Optional high-level evidence notes, if provided.
Do not provide
- Confidential details, incident narratives, or proprietary descriptions.
- Privileged content, legal strategy, or attorney-client communications.
- Personal data (names, personal identifiers, individual performance information).
Use limitation
- Responses are used only to generate your private Executive Risk Signal Report (PDF).
- Not shared, not resold, not used to train models.
Retention
- Stored for 30 days for delivery and re-issue, then deleted on schedule (or earlier on request).
Delivery
- Report delivered asynchronously by email from an institutional address.
Alternative intake path
- If web intake is not acceptable, request an invoice + private completion link.
